Intended use: For full-time undergraduate or graduate study at accredited  4-year or graduate institution in United  States.
Eligibility:  Applicant  must  be U.S. citizen.
Basis for selection: Major/career interest in health-related professions or  health  services administration.
Application  requirements: Transcript.
